Milan has 17 (!) train stations, but if you are reaching the city by train you will most likely arrive in Central Station, Lambrate Station, Cadorna Station or Garibaldi Station.
All of them are connected to the underground metro system, managed by ATM.
So jump off the train, reach the Metro station underneath, get a ticket and explore!
